Adult & Children’s Targeted Case Management
Targeted Case Management services are available for Adults and Children. This is a CARF accredited program that is designed to assist individuals in functioning successfully in the community. Case Management connects individuals, with Mental Health concerns, to the community services they need. The Targeted Case Manager and the individual work together to develop a service plan that identifies and prioritizes their service needs.

Services are provided to adults 18 or older and children 5-17 years old with a mental health diagnosis who are willing to participate in Case Management services.
Quality Life Center accepts Medicaid, Medicaid HMOs, and private pay.
Services are offered in Duval County, Clay County, and Nassau County
